Question: Can someone explain what that means? Does it mean that there is a bigger chance that the target will be affected by the next poison, or does it mean that the next poison will do greater damage?

Answer: next poison will have a greater magnitude for every negative (and positive!) effect it carries. Poisons always affect your target, however Bosmer and Redguards are 50% resistant to poison and vampires are completely immune (it still affects vampires).

On positive effects in a poison: For example a poison with "fortify enchanting" positive effect (Blue Butterfly Wing + Hagraven Claw + Hanging Moss) applied on self will have a greater "fortify enchanting" impact if you have "weakness to poison" status. Applying poison to self is tricky though. Making a loop is possible : applying wekness to poison (on self) - fortify enchanting (poison on self) - enchant stronger fortify alchemy gear - then make stronger poisons. This deserves a separate article really.

Instant effect in case of a double effect poison ?[]

Question: Does this effect applies instantly in case of a double or triple effect poison ? For example, mixing [River betty + Deathbell + Giant lichen] gives you a triple effect poison [Damage health ; Slow ; Weakness to poison]. Does the weakness apllies immediatly, thus increasing the damage health and slow effect OR does it only applies to the next poisoning of the same target ?

And how does it works in combination with the Concentrated Poison perk ("Poisons applied to weapons last for twice as many hits") ?

Answer: No, it doesn't work the way you described. The Weakness to Poison effect must be applied before applying the poison for it to deal increased damage.
That means if you applied the poison you described and then the same poison once more, the first one would deal it's normal poison damage, while the second one would deal increased poison damage. Because of this it may be beneficial to use more than one type of poison.
Personally I like to use the poison you mentioned as the first one against my target and after that I use another one which then benefits from the Weakness to Poison effect from the first one. The one I like is [River Betty + Imp Stool + Mora Tapinella] with the Damage Health and Lingering Damage Health effects.
