I completed the Ancient Power quest in Broken Oar Grotto with Serena as a follower. However, she wont leave the cave with me. I did another quest thinking she would respawn next to me but never did. I tried to go separate ways w/ her but she's in stuck inside the cave. Any way I can make her leave?

I had the same problem but all I did was go back inside and she followed me out. I haven't tried the Fus Ro Dah method yet. She doesn't become hostile if you attack or fus ro dah her. I've done it plenty of times when she would block a door way.
Yeah, I'm having the same problem, I tried everything. Reloading the game up, shouting at her, Leaving and coming back, Even Parting ways with her, which then She told me to meet her at Darkfall Cave then left. I checked there and Fort Dawnguard, and she was in neither place. She moves around fine in the cave, She's not physically stuck, she just cannot leave the cave.

Fast travel back to Broken Oar Grotto. The follower will now be standing outside the cave; if they are not, fast travel somewhere and they should appear."

This worked for me as well, with a one exception. After waiting 24 hours at Septimus Sigmus's outpost, I fast traveled back to Broken Oar Grotto, and Serana was not there. I then fast traveled to Winterhold and Serana was still not there. I then fast traveled to Solitude, and she appeared. So if the above doesn't work, try fast traveling to Solitude after fast traveling somewhere else first, or maybe just going straight to Solitude would have worked. Not sure if it will be consistent or not, but it worked for me.
Yeah, I had a similar problem. My followers would not leave the cave, only my Steadfast Dwarven Sphere would leave with me. However, my summoned Spectral Assasin, Cicero, and Serana did not want to leave. I went back several times and was able to get them to somehow appear at the outside entrance without even going in it, save for the Spectral Assassin; I just resummoned him (though he lost his bow/arrows that way). I'll have to use the Solitude jail method next time. -_-
I've not yet tried any of the above solutions, quite simply because what I do works every time. If you have Serana and/or another follower that won't leave Broken Oar Grotto, leave the Grotto and go to a location where you can go inside, say Breezehome or any other location where you can physically leave the open world. This doesn't apply to towns or cities, it needs to be a tavern, pub, store, anything with a door to open. Then go back outside whatever location you decided to go into, and fast travel back to Broken Oar Grotto. Every time this happens to me, Serana is waiting outside when I get back. Hope this helps some people.
PS3: The solution about commiting a crime (tried to rob some clothes on pickpocketing) on Solitude and paying the bounty (Thieves Guild members pay attention because the bribe option is the first, I was so tense that I clicked that one first) will spawn Serana. She delayed like 2 seconds to appear, but when she appeared I just cheered like a child. It works.

I believe the question they're asking is, if you can propose to/marry Serana, as the option to do so does not show up even with an amulet of mara in inventory. In response to the above question, I don't believe you can marry her.
